In Wildnote, the Survey Forms refer to dynamic and customizable groups of questions composed of different data types - text, numbers, dates, selection lists, photos, settings, and more. Basically, a Survey Form is the template used on the mobile device to fill out record attributes along with all the other rules and settings you configure. Survey Forms are always created on the Wildnote website, as the mobile device is only used for collecting data.

There are three ways to create survey forms; create a new survey from scratch, create a survey from a library template, or duplicate and edit an existing survey form. Let's get started.

Creating New Survey from Scratch 

  1. Go to PROJECT ADMIN > SURVEYS
  2. Click the NEW SURVEY FROM SCRATCH button.
  3. Start adding questions as described further down in the article.

Creating New Survey from Library

  1. Go to PROJECT ADMIN > SURVEYS
  2. Click the NEW SURVEY FROM LIBRARY button and see if there is a survey forms in the library that is close to what you need.
  3. If you find one, then click NEW SURVEY BASED ON THIS LIBRARY SURVEY and the system will create a duplicate of the library template.
  4. Rename the survey template by clicking on the SURVEY DETAILS tab to the left and editing the name field.
  5. Edit the survey template questions or use the template as is.

Creating New Survey from Duplicate

  1. Go to PROJECT ADMIN > SURVEYS
  2. Identify an existing survey similar to your needs. Click the DUPLICATE button to the right of the survey name.
  3. The new survey form will open in edit mode. Make sure you go to eh SURVEY DETAILS tab on the left to rename your new survey form. Otherwise you will have the default name of "Duplicate" at the end of the old name.
  4. Go back to the SURVEY LAYOUT tab to edit the survey template questions. 

Adding Survey Questions
Every survey starts with a date field that is automatically populated by the system.

  1. Click ADD QUESTION from the bottom of the survey form.
  2. Select the 'Question Type.' For example: “Text-Single Line." 
  3. Then name your question. In this example, you might say, “Team Member Name”. 
  4. Click SAVE and you have made your first question!

PRO TIP: There are over 20 different question types; you can learn about those by either looking to the helper text to the right of the survey creation window, or by clicking on the small “i” for more information about each question type. If you click on the small “i” for the helper text, just click it again to make the helper text disappear.

Don’t worry if you put a question in the wrong order - it is very easy to drag the questions to wherever you need to.
